Visit NSDL website. Click on ‘Online Application for Changes Or Correction in PAN Data (PAN Change Request Form)’
Scroll down to the end of the page and select ‘Body of individuals’. A form like ? will open.
Fill out the entire form. Choose the payment mode. And, select if you want only e-PAN or a physical copy as well.
Upload your signature and self-attested supporting documents such as ID proof, address proof and proof of birth. Hit ‘Submit’
You will get an application reference number. You can use it to check your application status.
– Marriage license or a marriage invitation card – Publication of a name change in a government publication – Passport photocopy with the husband’s name on it – Certificate from a registered official (just for the applicant’s name change)
